Before your first appointment please download, print , fill in and bring the 5 following forms to your first visit. 

New patient information

Please arrive 15 minutes before your scheduled appointment so we may add your information to our computer.  In consideration of our other patients, if you arrive 15 minutes past your appointment time you may be asked to reschedule.

If you are a minor we require the ID of the party responsible for payment.

Payments/Co-payments

Payments and co-payments, if required by your health plan, are expected at the time of service.  If you are hospitalized, you will receive seperate bills, one for our services and one for the hospital services. If you have surgery, you may also receive a separate bill for anesthesia services.

Insurance

LPW accepts most insurance plans. Please have your insurance or Medicare card available at each visit. If you do not have your insurance card with you, your appointment may be subject to rescheduling. If you do not have your insurace card with you, your visit will be considered "self-pay". Please call our office and ask for the billing department with any payment or insurance related questions.

Cancellation Policy

To help ensure that we are able to provide prompt, efficient service to our patients, please notify us of a cancellation at least 24 hours before your scheduled appointment. A cancellation fee will be charged for cancellations received less than 24 hours in advance.
